Arthritis.

Cartilage is a part of our shoulder that protects the shoulder joints during movements. Due to several reasons, the cartilage might wear down, leading to painful movement of the shoulders. Most of the aging causes such kind of wearing down. Not only might the pain shoulder arthritis also lead to shoulder stiffness and limited rotation and movements. 

Rotator cuff pain.

The rotator cuff is comprised of a rotator capsule, tendons, and rotator muscles. Due to excessive activities or age-related changes, these parts might get damaged. The occurrence rate of such a condition increases after the age of 30. However, this is not a serious condition. In around 40 percent of cases, the pain goes off with time. Anti-inflammatory medications and injections can treat the rest of the cases. 

While a few of the above conditions are easily treatable and even do not requires any treatments, rests like arthritis, spinal problems need immediate actions. Physiotherapies, exercise can treat the minor conditions well; the surgeries are not required if such interventions are applied at the very initial stages of major issues. The untreated spinal problem can cause permanent damage.

Long office hours, continuous sitting in the same prostitute in front of the office desk, causing various kinds of shoulder related problem. 3 out of my 5 patients come with different kinds of shoulder mobility and pain-related problems on daily basis.

Most of you might say, from a hectic daily schedule how can you manage time for such exercises. So Dr. Sushal Shanthakumar will discuss four exercises that you can do at your work. 

Shoulder rolls. It’s the easiest one.

  • First, sit straight on your back with tucked in the chin.
  • Then roll your shoulders forward, up, back, and down following a circular motion.
  • Repeat the same for 5 to 6 tine and follow the reverse motion for another 5 to 6 times.

Desk angels. Sitting straight in the chair is the perfect posture but I agree that’s not possible all the time. Desk angel helps to prevent back pains due to bad sitting postures.

  • Sit straight in the chair keeping your arm at shoulder level. Your arms should provide 90 degrees bend in your elbows.
  • Now keep your head and torso stationary and start moving your arms overhead until your hands reach towards the ceiling. At this point, your arms will be lining with your ears.
  • Then back to the starting position slowly.
  • Keep in mind, while you are doing this activity, your mid back has to feel some pulling. This helps your spines to relax.
  • Repeat the same for 8 to 10 times.

Upper trapezium stretch. This is the least time-consuming shoulder exercise you can do at your work.  

  • First, sit straight on your back.
  • Then tilt the head sideways toward the shoulder.
  • Now gradually try to make your stretch lager by dropping your shoulder blade on the reverse side toward the floor.
  • Hold the posture for 10 seconds.
  • Repeat twice on each side.

Armpit stretch. You might find this exercise funny. I would suggest when there is no one around you. Otherwise, you will get embarrassed. It’s like smelling your underarm. But believe me, this is one of the best exercises that protect you from shoulder pain.

  • First, sit and keep your back straight.
  • Rotate your head to take your nose directly above your armpit.
  • Now hold the backside of your head with your hand and push the head gently to take your nose closer to your armpit. This pushing should be continued until you feel discomfort.
  • Hold for 10 seconds and repeat twice for each side.

Now, come to a few of the other exercises that help you to keep your shoulders healthy. Although you might not be able to do these at work but investing 30 to 40 minutes for your weekend will be enough for these.

Standing arm swings.   This increases blood flow to the shoulder, making you joint stronger and mobile.

  • First, stand straight keeping your arm straight by your side.
  • Now engage your core and swing your arms straight in the forward direction up to the highest point you can go, but without raising your shoulders.
  • Then, move back your arms to the starting position and repeat the process for 40 to 60 seconds.

Downward dog pose. This is the best exercise for improving your shoulder strengths.

  • Lie down straight on your chest.
  • Now starts on your hands and knees.
  • Press your hand and lift your hip upward in the direction of the ceiling.
  • Bend your knees slightly while pushing your hip up. This will help you to spread the weight evenly into your hands and feet.
  • But never bend your spine. Always keep it straight.
  • Maintain the pose with straight spine and hip upward for 1 minute.

Cross arm stretch. This keeps your rotator cuff muscles healthy. If you do this exercise properly you should feel a good amount of stretch in your rear shoulder.

  • Stand straight and keep your feet slightly less than shoulder-width apart.
  • Now move your left arm up to a position little less than your shoulder height.
  • Place your right hand on your left elbow.
  • Now, gently pull your left arm across your body. While doing this support your arm with right arm.
  • Retain the position for 30 minutes, and then repeat for the opposite side.
  • Practice this 3 to 5 times for each side.

Post-operative shoulder cares are very critical. Most of the time I found patients become impatient due to delay in resuming their daily activity. 

The recovery time varies based on surgery type and injury but it’s also true that your wrong post-surgical care mistake might extend your recovery time.

First, let’s have a checklist of all the must-have things you need to buy.

Detachable shower head. You have to protect your incision from water for a certain period. A detachable showerhead is the easiest and safest way to do that.

Pump soap. In case one of your shoulder gone through surgery and use can use only one arm then pump soap is the best solution. However, if your both shoulders went through surgery, then you have no option to take the help of others for a certain time. Keep in mind, excessive mobility just after the surgery can harm your shoulder to a greater extent.

Shower chair. You should be more conscious about falling. Sometimes the pain killers and other post-operative medication might leave you dizzy. Risks of falling increase in such cases. A shower chair is an ultimate solution to handle such risks.

Waterproof bandages. As I told earlier you have to keep your incision dry until the doctor provides further instruction. Waterproof bandages do the job very well.

Now let’s discuss the care strategy. The complete healing of shoulder joints might take several weeks. For the first two weeks, extensive discompose and pain is very common. Even for more critical surgery, the pain might stay for 3 to 4 weeks. So your care strategy should include:

Ice Compression. If you feel extensive inflammation or pain, place and hold an ice pack directly on your shoulder for 20 minutes. Repeat this process in a 2 to 3 hours interval for the first two weeks of post-surgery. After this phase, you can start a heat pack. But don’t forget to consult your doctor before starting the heat pack.

Medication. As per the instruction of your doctors, you can take local anesthetics, opioids, or nonsteroidal anti-inflammatory drugs to get relief from extensive pain and swelling. 

Clothing and outfits. I recommend wearing loose-fitting clothes. Always wear such clothes that do not require lifting your arms over the head. Zippered or button-down shirts are a better option than the pullover ones. 

For women, try to use bras or inner garments that have the closing button on the front side. If you can’t find such a bra, then put the bra on the backward and clasp on the front side. Then twist it after closing the clasp.

In the case of shoes, slip-on is the best option as you don’t need to move your arm for binding the laces. 

 Sleeping position. I know it’s the biggest challenge every patient faces after the shoulder surgery. In the initial days, you have to sleep on the opposite site putting pillow support at your back. Latter days, you can lie flat but only after the pain and discomfort start to reduce. If possible try to set your bed against the wall and keep you back to the wall side while sleeping. I would suggest wearing a sling in the affected arm to prevent any kind of unnecessary movements.

Be patient to resume your physical activity. Most of the cases, I find people start heavy activities once their pain is gone. Never do such things. Although you don’t feel pain, still your injuries are not recovered completely. It’s always recommended to follow the doctor’s instructions for resuming your daily activities. It’s better to resume step-by-step. 

Shoulder surgery exercises. Once you get rid of pain and inflammation start some recovery exercises like shoulder forward elevation, neck flexion, shoulder internal rotation, and walk-up as per your doctor and physiotherapist recommendation. 

So these were the most basic guideline that will help to recover soon.

In this article, Dr. Sushal Shanthakumar will discuss ‘Anatomy of Shoulder’ structure and the most common conditions of shoulders. 

Among all our body joints, the shoulder is one of the largest and complex ones. Shoulder is made of two joints:

  • The acromioclavicular joint
  • The glenohumeral joint.

 Acromioclavicular joint. This joint connects acromion, a part of the shoulder blade and collarbone. Acromion is a part of the bones that projects scapula.

Glenohumeral joint. This connects the ball that is the humeral head and the socket that is the glenoid. You might be confused about the humeral head and glenoid. Humeral head nothing but the upper arm area from where your arm starts. The humeral head is comparatively loosely fitted to the joint making it more porn to injury. Glenoid is the end part of the scapula, from where the humeral head starts. 

Other significant bones and strictures of the shoulder include,

Coracoid process. A small hook-like bony projection from the scapula.

Rotator Cuff. It surrounds the shoulder giving it support and allowing for a wide range of motion. It’s made up of the combination of several muscles and tendons.

Brusa. Your shoulder contains a small sac of fluid protecting the tendons of the rotator cuff. This is called, Brusa.

Labrum. This is the cuff of tough but flexible tissue, called cartilage. Labrum creates a cup-like structure, into which the ball of the humeral head gets fitted. 

 Due to various structural condition of these parts and sub-parts of the shoulder might not work properly. At this point, you feel inflammation, pain, or stiffness in the shoulder.

Frozen Shoulder. Due to inflammation, the joints become stiff limiting your shoulder movement with ongoing time. At the initial stage, you face joint pains. 

Gout. Shoulder in the most uncommon part of the body where got can take place. However, this is not impossible yet. Gout is a sub-type of arthritis that leads to crystal formation within the joint. As a result, you experience inflammation and joint pain.

Rotator cuff tear. Any kind of sudden injury or excessive activity of your shoulder joint might tear any of your muscles or tendons covering the humeral head. This causes pain.

Rheumatoid arthritis.  This is another sub-type of arthritis, attacked by the immune system. You can experience occasional and sudden pain at the initial stage. The severity and frequency of pain increase over time. It also limits the immovability of your shoulder.

Osteoarthritis. This is the most common type of arthritis affected by aging. Lack of calcium deficiently might speed up the onset of this ortho problem and you get affected at an early age. Osteoarthritisattacked the knee mostly but the cases of shoulder osteoarthritis are in rapid growth.

Shoulder dislocationIf you feel pain or any kind of ‘popping’ sensation while raising your arms, it might be possible that either the humeral head or any other bond has slipped out from its position.

Shoulder tendonitis. This is the inflammation of tendons in the rotator cuff.

Shoulder bursitis. Sometimes you might feel pain while performing any overhead activities or experience pressure on the upper outer arm. The cause is the inflammation of the bursa.

Labral tear. Tears of labrum the cuff of cartilage, are the very common structural condition that occurs due to accidents or overuse of arms. But the good news is most of such tears heals without any surgeries.

So, apart from the aging issues and accidents, the rest of the structural condition, we can take care of by changing our lifestyle like a healthy and balanced diet, walking. Regular physical activities can even slow the onset of osteoarthritis.

The average adult gets one to six time colds every year, with symptoms lasting a week to 10 days. Should you let these colds stop your exercise routine? Probably not, as long as you pay attention to what your body tells you.

The general rule for exercising with a cold is that if symptoms are all above the neck, it is safe and sound to exercise. In other words, if your only symptoms are

  • Runny nose
  • Stuffy nose,
  • Watery eyes
  • Mild sore throat,

Then go ahead and exercise, but keep your routine low to moderate in intensity and briefer than usual. Studies have revealed that light-to-moderate exercise has no effect on a cold—it neither prolongs nor shortens the duration of symptoms. High-intensity exercise, on the other hand, has a negative blow on the immune system and may prolong cold symptoms.

If you have any below-the-neck symptoms such as a

  • Cough
  • Chest congestion
  • Fever
  • Vomiting
  • Diarrhea
  • Body ache

Then it is time to take a few days off. It is especially important to stop exercising if you have a fever because fever is an indication that the body is fighting an infection. When you have below-the-neck symptoms, support your immune system by getting plenty of rest, staying hydrated and eating nutritious food. Return to exercising gradually once symptoms disappear.

Although it is safe to exercise with a cold, please consider the health of your fellow exercisers. Cold viruses are easily transmitted and can live on hard surfaces such as exercise equipment for an hour or more. As a courtesy to your colleagues, try changing your workout to one that uses little or no equipment.

  • Regularly icing the affected area – Icing help in reduces swelling. The ice pack can be covered with a towel or cloth to prevent ice burn.

Hospitalization is required for non-surgical treatment as to monitor the pain or any unusual symptoms. As the treatment proceeds, the doctors may suggest for physiotherapy treatment if required

Most of the time scapular body fractures are typically treated non surgically with good results, but in case of fractures of the scapular neck or glenoid may require surgical intervention

Surgical Treatment

Dr. Sushal Shanthakumar says, In some of the cases, surgery to treat a broken scapula becomes necessary. Surgery for a scapular fracture may be required when:

  1. Angular deformity – In this condition, the angle of the bone causes a deformity in the limb.
  2. Displacement – In this condition, the bone positioning is no longer in its correct, anatomical position.
  3. Surgery is required if the glenoid socket is fractured, causing instability of the ball and socket joint.
  4. Surgical intervention is required if ant additional bones, such as the clavicle, are fractured.

Based on the condition physician may recommend for a physical therapist for rehabilitation.

As we know that any surgery poses some risk, such as infection, bleeding, or nerve and blood vessel damage. In most of the cases, people heal from scapular surgery with good overall outcomes.
